In order to have your patient test populate the lab result document correctly in athenahealth, your analyte naming must match in both the relaymed test type configuration and the athenahealth office order type configuration. For example:

Note: the order of the analytes doesn't matter, and it's case-insensitive.
If the analyte names do not match up you'll find that both the analyte spellings will be on the lab result document. However, only the analyte matching the spelling configured in myrelaymed test type configuration will be populated with a value. For example, notice in the following screenshots we have incorrectly spelled the blood analyte in the relaymed portal:
